A calcaneal osteotomy is a controlled break of the heel (calcaneus) bone and shifting it inside (medial) or outside (lateral) to correct deformity of the foot and ankle.  By shortening or lengthening the heel bone the goal of this procedure is to align the heel so as to influence how weight-bearing stress is applied to the foot, ankle, knee and hip. This procedure is often recommended for patients who suffer from flatfoot deformity or abnormally high arches and where other non-surgical procedure have not produces any significant results.

Calcaneal osteotomies may be combined with other procedures, such as tendon transfers.
